Breed Survey Instructions
To experience the greatest success using this online entry form, we suggest the following:
  • If possible use a Chrome browser for the best experience. If at all possible DO NOT use Internet Explorer as it MAY not work.
  • If uploading pictures of your documents taken with your phone, please do the following. Be sure that they are clear and all information can be read. If your phone has the option to compress or make them smaller, please choose that option. Standard photos on new phones are very large because of their high resolution and may cause time outs while trying to upload them.

Didn't have all of the documents you needed to upload when you entered your dog?  Have more documents to upload?  Then click the link below to go to a new screen where you can verify what you have already uploaded and upload any additional documents at any time while entries are open.
  • Select the type of Breed Survey
  • The "Registered Name of Dog" field is a list pre-populated with dog names.  As you type in a dog's name the list will display matches.  If the dog you are entering is in the list click on the appropriate dog's name, if your dog's name is not in the list, then type in the dog's name. Press tab to update fields with dog information.
  • For dogs that are in the list, when their name is selected any fields of information that are available will be completed.  All of these completed fields can be edited.
  • For dogs, sires or dams that have more than one working title, keep selecting the earned titles from the appropriate list.  If a dog has an IPO3 and FH1 title, click IPO3 from the list and the title will be displayed in the box to the left of the list.  Then click FH1 from the list and both titles will be displayed in the box to the left of the list
  • In the Owner and Co-owner sections, if you type in the zip code, for a city in the United States or Canada and hit either the Enter or Tab key, the city, state and country for the entered zip code will be completed automatically
  • Once the Submit button has been clicked all fields will be validated.  If there are missing or incorrect fields, those errors will be displayed and you need to correct them and resubmit the form
  • The form can be printed by clicking the print button.  The preferred option would be from the Print option is to save as a .pdf document if you have that option from your printer.
  • If an email address was entered in the owner email field, an email message will be sent confirming the entry was submitted
  • Once the entry is successfully submitted, you will be taken to the Entry Information Page.  Here you will receive confirmation that your entry was submitted.  You will be provided payment options, where available.  You will also have the opportunity to upload all required documents for your dogs' entry.

We, at the Utah GSD Schutzhunde Club, would like to welcome you to our Event.
We trust that you will find as much pleasure in the sport as do we. We would like to share our required respect and sportsmanship standards, which we expect of both participants and spectators at all our Utah GSD Schutzhunde Club events. These standards of behavior pertain to conduct both on and off our field. Confrontational and unsportsmanlike behavior in any form will not be tolerated; e.g., verbal, physical, social media, and e-mail.

  • Punitive or inhumane treatment of any kind toward an animal during a sanctioned Utah GSD Schutzhunde Club. event will not be tolerated.  This standard of treatment includes practice, trials, seminars and all other events held at the Utah GSD Schutzhunde Club training facility.  Such acts will be reported to United Schutzhund Club of America (U.S.C.A) and punishment will be enforced to the fullest extent as per U.S.C.A. bylaws.
  • All judge’s decisions are final and are not subject to debate.  Our judges are our esteemed guests and should be treated with respect at all times.

In an effort to make the trial experience enjoyable for all involved, we have established some trial rules

that must be followed by all trial participants.

  • During trials, all pairings are final. If you have a pairing concern, it must be made at the time of entry or at least one week prior to trial. Please make that concern known either by email to the secretary / contact person or indicated on the entry form.
  • All entry forms and money must be mailed or hand delivered to the trial secretary by the cutoff date listed on the entry form.
  • Your entry will be confirmed per email when a completed entry form and money is received by the trial secretary.

Copies of the following documents must be submitted with this entry.
The original documents must be presented to the Breed Survey Secretary at check in:

  • You must upload or email copies of all of the dog's required documents.
  • USCA, AKC, SV, CKC or other FCI recognized club registration from the dog's country of birth.
  • USCA, AKC, SV, CKC or other FCI recognized 4-generation pedigree with SV a-stamp or other WUSV accepted hip & elbow certification.
  • For a USCA Breed survey the dog must be USCA registered with SV a-stamp or other WUSV accepted hip & elbow certification.
  • Show Card with at least a "G" rating awarded by a USCA, SV or other WUSV accepted judge in a regular class over 12 months of age.
  • Original Körschein for Breed Survey After Interruption, Lifetime/Re-Survey and Upgrade.
  • Original scorebook for First Time Breed Surveys and Upgrades indicating passed AD and BH, and at least one IGP, IPO or SchH class title earned under a USCA, SV or other WUSV accepted judge with at least 80 in the C phase, or an HGH.  Dogs with an HGH are not required to pass the AD or BH.
  • Original proof of change of ownership.
Please note, Dogs owned or co-owned by a German resident are not permitted by
the SV to participate in breed surveys in the United States or Canada.
  • NO ENTRY FEES WILL BE REFUNDED
  • Entries for which payment has not been received will not be accepted
  • Entries that are not received on or before the event deadline date will not be accepted
  • You MUST click the "Submit Entry" button to submit your entry.
  • Click the "Print" button to print a copy of your entry.
